Welcome to Clear Lake Dermatology and thank you for making an appointment with us.

Our staff looks forward to meeting you and discussing your goals. We strive to keep our appointments on time as scheduled. Prior to your appointment, we encourage you to follow the link emailed to you and fill out each tab to the left in the sequence prior to your arrival. This will help to expedite your visit. Please keep in mind this link will expire within 24 hours. If for some reason you do not wish to provide us with an email address, then please print and fill out the document below that is titled Patient Registration Form, and bring it with you to your scheduled appointment. If you are unable to download our patient forms, then it is recommended that you arrive at your appointment 15 – 30 minutes early so you can have ample time to fill out the appropriate forms.

What to Bring to Your Appointment

Please bring proper identification and insurance card(s) to your appointment. All HMO insurance plans must have a primary care physician's written referral prior to your visit. The written referral can be brought in or faxed to our office.

Minors

A parent or adult guardian must accompany all patients under the age of 18. State law requires that the parent/guardian give final consent authorizing us to treat a minor.

Cancellation/Missed Appointment Policy

If you cannot keep an appointment, kindly give at least 48 hours advance notice to cancel and reschedule so that other urgent appointments can be scheduled in your place. There will be a charge for missing confirmed appointments.
